In order to qualify for the Home Help program, you must have active Medicaid, HAP, Aetna, Molina, Humana or Priority Health insurance. Be over the age of 18 and require physical assistance with at least 3 activity of daily living. An adult services worker will complete an assessment in your home to determine whether you qualify for assistance through the Home Help program.
